2. Registration & Consent Forms


PARENTAL / LEGAL GUARDIAN CONSENT
• I agree to my son/daughter taking part in the activities of the club.
• I confirm that I have legal responsibility for the children linked to my club account and that I have informed them of the content of this form.
• I understand that I will be kept informed of these activities – for example, timing of training/matches and information concerning the club.
• I understand that in the event of any injury or illness all reasonable steps will be taken to contact me or the alternative contact and I give permission for the club to administer any appropriate or necessary medical attention.
• I consent to the club maintaining confidential records and information relating to myself and/or my child for the purpose of the club administration and to facilitate my child’s full participation in Claygate Cricket Club activities. I understand that I have the right to see this information if I so desire, providing I give appropriate notice to the club. I understand that details of how this information is used are covered in the club’s Privacy Notice which can be found on the website www.claygatecricket.co.uk in the Information section and is GDPR compliant.
• I understand that junior membership of the club also provides a parent/guardian with honorary membership of the club; this does not give entitlement to playing rights but does give one adult voting rights at the club’s AGM or EGM. For more details please refer to the Information section on our website.
• I agree to abide by the club’s Code of Conduct for Members & Guests and that my son/daughter will be made aware of & understand the importance to abide by the club’s Code of Conduct for Young People. Both codes are available in the Information section on the website.
• I confirm that to the best of my knowledge all the information provided is accurate & I will inform the club of any changes to this information in a timely manner.
